    a. Updated CV (Curriculum Vitae)
    b. All the education Degree/Diploma/Certificates.
    c. Provisional Certificate / HEC Card(s)
    d. All the experiences Certificates till the date of applying.
    e. PMC/PMDC Registration Certificate (if any fee occurs).
    f. National Identity Card.
    g. Domicile.
    h. 3-Fresh passport size colored photographs.
    i. If the applicant has foreign degree must attach HEC equivalence certificate.
    j. Non-refundable bank draft of Rs. 1,500/- (only for serial numbers 1 to 15 only) in favor of the Director General Administrator, PHOTA, Lahore. Applications for these posts will not be considered without the prescribed bank draft.
